Reporting

We are committed to lightweight reporting that captures your impact without putting a burden on you or your colleagues. Here’s what we ask for: One midpoint project report. One endpoint project report. That’s it.

Midpoint project report

Due 15 business days after the project’s midpoint date

Endpoint project report

Due 15 business days after the project’s end date

Your project’s midpoint and endpoint dates are confirmed when you are selected as a grantee. Reach out to us if you are not sure when your reports are due.

Both reports should include:

Short narrative responses

Tell the story of your program and how it is impacting the community.

Quantitative data

Share data related to your project like COVID-19 tests distributed, number of flyers printed, number of people who attended an event, etc.

Photos

We encourage photos (with permission of photographers and subjects) but they are not required.

Invoicing

The grant you receive from us is paid out as reimbursements for your program-related expenses. To get reimbursed, you must submit invoices via the online form.

Here’s what to know about invoicing:

Invoices must be submitted on the 15th or 30th of each month. 

You must submit invoices and documentation via our online form.

If you miss a submission deadline, your invoice will be included in the next batch.

We batch and send invoices to DOH for approval every two weeks.

Once we send your invoice to DOH, you can expect your reimbursement within 30 days.

For all expenditures over $1000, there must be documentation submitted with the invoice.*

Documentation for payroll expenses must include hours worked and hourly rate for each employee. You do not need to include names.

If seeking reimbursement for stipends, include the recipient’s initials and the number of stipends received during the invoicing period.

*Examples of acceptable documentation include: screenshots of bank statements, credit card statements, payroll reports, and scanned receipts.

Invoicing Form

Metrics tracker template

Track metrics related to your outreach activities, educational programs, clinics, and other events.

Feedback channels

We want to build a culture of trust and transparency with our partners. We highly value your feedback on everything from the application process to technical support to the reimbursement model and more.

There are several channels for feedback so you and your team can update us on your experience as you move through the process.

Here are the feedback channels you can use:

CDO Program Feedback Form

Share your opinions, insights, or questions at any time using this form. If you prefer, you can choose to keep your comments anonymous. We will respond to these submissions if appropriate or requested by the submitter.

Midpoint & Endpoint Project Reports

On both reports, there are optional questions about project implementation and the support you have received from us. See more on required reporting.

Optional end-of-project feedback survey

Shortly before the end of our grant program, we will reach out to all our partners to request an interview about their experience and ask them to complete a survey.  All partners will be invited to participate in these optional activities.
